Outcome-driven care

Clinical Specialties oversees the development, implementation and evaluation of clinical programs delivered by our network home care agencies and/or our own staff. Many of the programs are in conjunction with some of the major payers in Ohio and the surrounding areas.
  • Care Transitions Program
    • Reduce Hospital & ER re-admissions
    • Increase patient self-care management of chronic conditions
    • Provide clinicians with the education and tools to better support their patients learning and confidence of their health
    • Data management and outcome reporting

  • TPN Case Management
    • Experienced TPN infusion RNs case manage a patient from time of referral to post DC of TPN therapy
    • One point of contact for referral sources, home care agencies, payers, physicians, patients and caregivers via the TPN Case Manager RN
    • CSI clinical team of TPN Case Manager RN, Pharmacist, & Registered Dietician collaborate with the patient's physician, home care agency, patient and caregivers to provide excellent clinical service
    • Data management and outcome reporting

  • IV Nursing Partnerships
    • For agencies who have dedicated infusion teams/nurses; able to provide a wide range of infusion services; both adult and/or pediatric patients
    • For those able to consistently staff IV cases in rural/remote areas

  • Clinical CEUs and Training Assistance
    • Over 15 one-hour CEUs approved for nursing, CCCM, and social workers
    • CEUs provided live/ on-site or via webinar by our Clinical Programs Department staff
      • Topics include infusion- related, disease management of chronic conditions, communication and learning/education strategies
    • Provide clinicians with the education and tools to better support their patients learning and confidence of their health
    • Data management and outcome reporting

  • On-site clinical training and support is by our Nurse Educator who assists agencies with training and updates on various infusion devices, products, and clinical conditions.
    • Can be provided live, by phone, webinar and/or Skype
    • Assist network agencies with infusion competency testing/fairs
